At the options screen, hold L1 + L2 + R1 + R2 and press X, Up, X, Up, Down, Square to unlock everything.

Unlock Extra 2 Option
Complete and save the game using the 'hard' difficulty setting to unlock the 'Extra 2' option at the main menu.
